HCPCS G0482 is officially defined as "Drug test(s), definitive, utilizing (1) drug identification methods able to identify individual drugs and distinguish between structural isomers (but not necessarily stereoisomers), including, but not limited to gc/ms (any type, single or tandem) and lc/ms (any type, single or tandem and excluding immunoassays (e.g., ia, eia, elisa, emit, fpia) and enzymatic methods (e.g., alcohol dehydrogenase)), (2) stable isotope or other universally recognized internal standards in all samples (e.g., to control for matrix effects, interferences and variations in signal strength), and (3) method or drug-specific calibration and matrix-matched quality control material (e.g., to control for instrument variations and mass spectral drift); qualitative or quantitative, all sources, includes specimen validity testing, per day; 15-21 drug class(es), including metabolite(s) if performed." Ambetter of North Carolina's June 2026 provider newsletter says it will stop covering 48 HCPCS H codes beginning January 1, 2027, and will cover H0006 from that date. The newsletter also says drug testing codes G0482 and G0483 are no longer reimbursable under the drugs-of-abuse policy (no date is given), and it lists clinical policy revisions effective August 15, 2026 and a reminder to verify member eligibility by paid-through dates.
Health Net will no longer reimburse CPT codes 80320-80377 for definitive drug testing effective October 1, 2026. Providers must transition to HCPCS G-series bundled laboratory codes (G0480-G0483) to avoid claim denials. The policy particularly affects mental health providers with specific drug testing code bundling rules using a 7-day lookback period.

The policy reinforces that only CPT codes 80305, 80306, 80307 and HCPCS codes G0480, G0481, G0482, G0483, G0659 are covered for drug testing in substance abuse and pain management; CPT codes 80320-80331, 80345-80354, 80356-80365, 80367-80373, 80375-80377, and 83992 remain non-covered. Drug testing is limited to 24 dates of service per calendar year.

Maryland Medicaid clarifies that MCOs (not the Behavioral Health Organization) are responsible for reimbursing toxicology testing performed by pain management providers in their own offices. Excellus BlueCross BlueShield policy 2.02.50 for urine drug testing considers in-office or point-of-care presumptive immunoassay urine drug testing medically appropriate for outpatient pain management and for outpatient substance use treatment, both for baseline screening and for ongoing monitoring at a frequency matched to the patient's risk. Definitive urine drug testing is considered medically appropriate when presumptive testing cannot identify a specific substance or drug, when a definitive drug concentration is needed to guide management, to rule out an error as the cause of a qualitative result, or as part of a differential assessment of medication efficacy, side effects, or drug-drug interactions. The October 22, 2026 version is recorded as an annual review with the policy intent unchanged.

Blue Cross Blue Shield Association policy 2.04.98 added CPT 0603U, a presumptive drug assay of 77 drugs or metabolites in urine by LC-MS/MS reported as positive or negative, which the code table lists as new as of 01/01/2026 and as investigational. The policy considers routine presumptive urine drug testing not medically necessary in chronic non-cancer pain management and substance use disorder treatment, and considers hair drug testing and oral fluid drug testing investigational in both settings. Presumptive urine drug testing is medically necessary only for the baseline screening and monitoring frequencies the policy lists for each setting.

Aetna considers presumptive urine drug testing medically necessary for people in a chronic pain or substance use disorder program when an indication applies, such as starting treatment in the program, suspected illegal or non-prescribed substance use, suspected overdose, monitoring of chronic opioid therapy, or testing documented as part of an active treatment plan. Definitive or confirmatory urine testing is medically necessary only for people who meet the presumptive testing criteria and also have a listed indication, such as no commercially available presumptive test, an inconclusive or inconsistent presumptive result, or a disputed presumptive result. Testing ordered by courts, schools, employers or other third parties is considered not medically necessary.
Aetna's clinical policy bulletin on drug testing in pain management and substance use disorder treatment states that presumptive urine drug testing is medically necessary for persons starting treatment in a pain management or substance use disorder program, for persons on chronic opioid therapy who are being monitored for chronic pain, and for suspected overdose, among other listed indications. Definitive testing is medically necessary only when a listed condition applies, such as an inconclusive or inconsistent presumptive result or a disputed presumptive result, while standing orders, broad panels, same-day testing of the same drug from two specimen types, and testing ordered by third parties are considered not medically necessary. The policy was last reviewed 09/01/2026 and its next review date is 10/08/2026.
MDHHS covers HCPCS codes G0477 through G0483 effective January 1, 2016, and covers 81528 for Healthy Michigan Plan only from that date. MDHHS covers L2785 effective April 1, 2016, and discontinued coverage of 76140 and G0464 effective April 1, 2016. Manual updates also cover behavioral health services, applied behavior analysis for children under 21 with autism spectrum disorder, and hospice payment rates for dates of service on or after January 1, 2016.
Acupuncture services under the HCBS-CIH waiver use CPT 97810 with modifiers U1 and SC for support planning and prior authorizations, effective September 6, 2023, and claims billed with 97810, 97811, 97813 and 97814 with U1 and SC reimburse against the approved prior authorization. Laboratory prior authorization is no longer required for CPT 81220 effective September 1, 2023, and an NCCI edit effective July 1, 2023 prohibits presumptive tests (80305, 80306, 80307) and definitive tests (G0480, G0481, G0482, G0483, G0659) on the same date of service, with a revision effective October 1, 2023. Child Health Plan Plus members are assigned to one of four managed care organizations, and providers must bill the MCO directly for services on or after the MCO enrollment start date.
Procedure codes 96136 and 96137 receive a reversal of a reduction to 100% of Medicare as of July 1, 2023, and the 3% across-the-board increase applies retroactively to that date. Claims past the 365-day timely filing period from the date of service must reference the previous Internal Control Number, and the next submission must be received within 60 days of the last action. Observation stay caps on units and billed hours were removed effective October 30, 2023.

Presumptive (immunoassay) urine drug testing in pain management is medically necessary with documentation of a baseline screening at treatment start or subsequent monitoring at a frequency matched to the individual's risk level, plus evidence the prescriber checked the state controlled substance abuse database at the start of each new course of treatment and at least quarterly. In substance use disorder treatment, presumptive testing is medically necessary at baseline (one time per program entry), targeted weekly for a maximum of 4 weeks in the stabilization phase, or once every 1 to 3 months in the maintenance phase, with the same database check, and definitive (confirmatory) urine drug testing is medically necessary only when immunoassays for the relevant drugs are not commercially available and definitive drug levels are required for clinical decision making. Hair testing and oral fluid drug testing are considered experimental or investigational in the treatment of pain management or substance use disorder(s) when any one of the five listed criteria is met.
