Summary
AHCCCS implemented multiple billing changes effective August-September 2026: (1) H2038 (Skills Training and Development) now requires prepayment review with mandatory supporting documentation starting 8/24/2026; (2) New Alivia 360 analytics system will begin denying NEMT claims with trip ticket errors and allergen serum prep claims without injection starting 9/4/2026; (3) SNF claims incorrectly denying with H189.1 remark are being automatically reprocessed. Billing teams must attach clinical documentation before claim submission and ensure all trip tickets and NEMT prior authorizations are accurate.
Action Required
By August 24, 2026: (1) Billing team must update workflow to require providers attach comprehensive assessment, treatment plan, and progress notes with EVERY initial H2038 claim submission through EDI Solutions portal—claims submitted without documentation will be denied. (2) Ensure all H2038 claims are submitted with Attachment Control Number (PWK) generated during initial claim submission; if PWK not created, use 12-digit AHCCCS claim number. (3) Verify all identifiers match exactly between claim and attachment to prevent mismatches. (4) Bill H2014 instead of H2038 when services are under 5 hours. (5) Apply HQ modifier when services involve 2+ individuals (group setting). Before September 4, 2026: (6) Audit all NEMT trip tickets submitted to ensure prior authorization is on file for trips over 100 miles and TN modifier is not used in urban areas; incorrect trip tickets will result in claim denials. (7) Billing software must be configured to flag and prevent submission of allergen serum prep claims without corresponding injection claims. (8) Verify SNF claims with H189.1 denials are being automatically reprocessed by AHCCCS—do not resubmit manually.
