Summary
Effective immediately, HCPCS codes G0482 and G0483 (definitive drug testing with 15-21 and 22+ drug classes respectively) are no longer reimbursable under the updated Clinical Policy CP.MP.50. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must stop billing G0482 and G0483 and transition to alternative codes or obtain clarification on covered testing options.
Action Required
By June 10, 2026: Billing team must immediately cease billing HCPCS codes G0482 and G0483 for drug of abuse testing. (1) Update billing software and superbills to remove G0482/G0483 from available codes or block submission; (2) Redirect all definitive drug testing orders to codes G0480 and G0481, ensuring medical necessity is documented in the patient record; (3) Contact Provider Relations Representative (referenced in policy) to obtain guidance on alternative testing codes or procedures that remain covered; (4) Notify providers and lab staff of the change to prevent claim denials. Claims submitted with G0482/G0483 will be denied as non-covered services. Implement this change immediately given the 1-day age of the policy.
