Summary
Starting April 1, 2026, Medi-Cal will no longer cover high-level definitive drug testing codes G0482 (15-21 drug classes) and G0483 (22+ drug classes) as they are deemed not medically necessary. Providers must use G0480 (1-7 classes) or G0481 (8-14 classes) with prior authorization for definitive drug testing.
Action Required
Before April 1, 2026: Billing team must update billing software to block codes G0482 and G0483 for Medi-Cal patients and redirect to G0480 or G0481 based on number of drug classes tested. Update prior authorization workflows for G0480 and G0481. Train providers on new limits - no more than 14 drug classes for definitive testing. Update encounter forms and EMR templates to reflect code changes. Claims using discontinued codes will be denied after April 1, 2026.
