Summary
Effective immediately (06/17/26), HCPCS codes G0482 and G0483 for definitive drug testing are no longer reimbursable under the Clinical Policy CP.MP.50. Codes G0480 and G0481 remain covered when medical necessity is documented. Billing teams must stop submitting claims for G0482 and G0483 and update internal systems accordingly.
Action Required
Effective immediately (06/17/26): Billing team must update billing software to deny or reject claims for HCPCS codes G0482 and G0483. Configure system to prevent submission of these codes. Notify all providers that G0482 and G0483 are no longer reimbursable; providers should discontinue ordering these tests or transition to alternative covered testing methods (G0480 or G0481 when medically necessary). Update encounter templates and lab requisition systems to remove G0482/G0483 options. Review any claims submitted in the past 30 days; resubmit claims using G0480 or G0481 if applicable and medically justified. Document policy change in compliance training materials.
