01Alaska providers must use the new standard fax coversheet for all travel service authorization requests.
02Submit non-urgent travel requests via fax (907.644.5982 or 877.438.8942) at least 10 days prior to travel.
03Reserve phone requests only for urgent travel needed within 3 business days.
04Incomplete fax requests will be returned after May 1.
05Update staff workflows to include proper fax procedures and coversheet requirements.
