01Alaska providers must transition to new travel authorization forms available at alaska.gov.
02Download and begin using the consolidated Travel Service Authorization Request Form, updated Transportation Authorization Update Form, and new Plan of Care Guidance.
03For local travel requests, only submit month and total visits per month - no longer need scheduled dates and times for each appointment.
04Update fax cover sheets and ensure staff are trained on new documentation requirements.
