Action Required
By June 11, 2023: Alaska providers must transition to new travel authorization forms available at alaska.gov. Download and begin using the consolidated Travel Service Authorization Request Form, updated Transportation Authorization Update Form, and new Plan of Care Guidance. For local travel requests, only submit month and total visits per month - no longer need scheduled dates and times for each appointment. Update fax cover sheets and ensure staff are trained on new documentation requirements.
