Summary
The Indiana Health Coverage Programs (IHCP) identified fee-for-service claims that incorrectly denied between October 23-25, 2023 with EOB codes 4021 and 4801. These claims are being automatically mass adjusted or reprocessed, and providers will see corrected payments on upcoming remittance advices with specific ICN prefixes.
Action Required
Immediately: Billing team should monitor upcoming remittance advices for reprocessed claims with ICNs beginning with '80' (reprocessed denied claims) or '52' (mass replacements). Review any waiver claims from Oct 23, 2023 or medical service claims from Oct 24-25, 2023 that previously denied with EOB codes 4021 or 4801 to confirm they are being automatically corrected.
