Update prior authorization rules to remove PA requirements for dental codes D2710, D2712, D2740, etc. and add PA requirement for D3999. Stop billing discontinued codes 0324U, 0325U, C1834, J0610, J0611 after March 31, 2023. Add new modifiers JK and JL to system. Review provider type restrictions for each new code before billing.
