Effective date

Aug 14, 2026

Summary

Aetna considers intensive cardiac rehabilitation programs (the Benson-Henry Institute Cardiac Wellness Program, the Ornish cardiac treatment program and the Pritikin Program) medically necessary as an alternative to traditional Phase II cardiac rehabilitation for members who meet the cardiac rehabilitation criteria, limited to 72 one-hour sessions, up to 6 per day, over a period of up to 18 weeks. Virtual delivery by real-time audio and video (excluding audio-only) is considered medically necessary when the telehealth program and member criteria are met, but hospital outpatient departments are not eligible for virtual telehealth services. Intensive cardiac rehabilitation is considered experimental, investigational or unproven for diabetes, hyperlipidemia, prostate cancer, metabolic syndrome and all other indications where effectiveness has not been established.

01Already effective January 1, 2026 (CMS rule changes described in the policy background): ICR codes are on the Medicare telehealth services list for physician office-based programs that use real-time, continuous audio-visual technology, and audio-only communication is not permitted.
02Hospital outpatient departments remain prohibited from providing ICR virtually.

04Billing team: bill G0422 (with or without continuous ECG monitoring with exercise, per session) and G0423 (without exercise, per session) only when the cardiac rehabilitation selection criteria are met, and confirm the member meets them before billing.
05Limit ICR to 72 one-hour sessions, no more than 6 per day, over up to 18 weeks.
06For virtual ICR, use real-time audio and video only.
07HCPCS S0340, S0341 and S0342 are listed as not covered for the indications in the policy.
08Charges for the program as a package, or for services not normally covered such as frozen food products, yoga or meditation, are not eligible for reimbursement, while identifiable charges for individual covered services such as office visits or diagnostic testing may be eligible subject to plan design.
