Additional CR services are considered medically appropriate after another qualifying cardiac event, such as another documented MI, another cardiovascular surgery or percutaneous intervention, or new clinically significant coronary lesions documented by cardiac catheterization. Phase III maintenance programs are considered not medically necessary, and intensive cardiac rehabilitation programs, including the Ornish Program, the Pritikin Program and the Benson-Henry Institute Cardiac Wellness Program, are considered investigational for all indications. The 10/22/26 revision is an annual update, and the policy intent is unchanged.

Phase II cardiac rehabilitation may be offered to patients with qualifying cardiovascular diagnoses, including myocardial infarction, coronary artery bypass graft, angioplasty or stent, arrhythmia, and severely depressed left ventricular function with ejection fraction below 30 percent. Formal cardiac rehabilitation should be initiated within 3 months after hospital discharge, and covered visits are 12 for low risk, 24 for moderate risk and 36 for high risk. Formal services must be conducted at a facility with current AACVPR certification, and prior approval is required for Fully Insured and most Self-Insured Groups.
Aetna's Clinical Policy Bulletin 0586 sets medical necessity criteria for heart transplantation and related services. Heart transplant is considered medically necessary for the listed indications, including cardiomyopathy, end-stage ventricular failure and congenital heart disease, when the member meets the transplanting institution's protocol eligibility criteria, and off protocol when all listed selection criteria are met. The policy also covers the FDA-approved total artificial heart as a bridge to transplant, AlloMap testing for rejection monitoring more than 2 months after transplant, and ex vivo heart perfusion for preservation and transport of the donor heart, and it lists several rejection tests and other procedures as experimental, investigational or unproven.
Aetna considers intensive cardiac rehabilitation programs (the Benson-Henry Institute Cardiac Wellness Program, the Ornish cardiac treatment program and the Pritikin Program) medically necessary as an alternative to traditional Phase II cardiac rehabilitation for members who meet the cardiac rehabilitation criteria, limited to 72 one-hour sessions, up to 6 per day, over a period of up to 18 weeks. Virtual delivery by real-time audio and video (excluding audio-only) is considered medically necessary when the telehealth program and member criteria are met, but hospital outpatient departments are not eligible for virtual telehealth services. Intensive cardiac rehabilitation is considered experimental, investigational or unproven for diabetes, hyperlipidemia, prostate cancer, metabolic syndrome and all other indications where effectiveness has not been established.
Aetna considers medically supervised outpatient Phase II cardiac rehabilitation medically necessary for selected members when a physician individually prescribes it within a 12-month window after a listed qualifying event, such as an acute myocardial infarction within the preceding 12 months, coronary artery bypass grafting, percutaneous coronary intervention, heart valve replacement or repair, heart or heart-lung transplantation, or stable chronic heart failure with an LVEF of 35% or less and NYHA class II to IV symptoms. The program allows up to two 1-hour sessions per day, for up to 36 sessions over 36 weeks, and additional cardiac rehabilitation sessions are considered medically necessary when the member has an additional qualifying event, up to a total of 72 sessions. CPT 93797 and 93798 are covered if selection criteria are met. Phase III and Phase IV cardiac rehabilitation programs are not covered under standard Aetna benefit plans.
Independence Blue Cross updated MA10.002g to MA10.002h, revising language regarding patient ability to switch from Intensive Cardiac Rehabilitation (ICR) to Cardiac Rehabilitation (CR) programs to align with applicable Medicare coverage documents.
