Transmyocardial and Endovascular Laser Revascularization (CPB 0163, reviewed 2026-03-20)
Aetna's Clinical Policy Bulletin 0163 considers open chest and thoracoscopic transmyocardial laser revascularization (TMLR) medically necessary for the treatment of medically refractory, severe intractable angina, when the appendix selection criteria are met. Excimer laser coronary angioplasty for all indications, percutaneous TMLR for refractory angina and all other indications, and TMLR plus cell therapy with adipose derived stromal cells or autologous bone marrow cells for ischemic heart disease are considered experimental, investigational or unproven because their effectiveness has not been established. TMLR by thoracotomy (CPT 33140 and 33141) is listed as covered for ICD-10 angina pectoris codes I20.1 through I20.9 when the selection criteria are met.
