Summary
Government-owned ambulance providers in Illinois must submit updated cost reports and signed Intergovernmental Agreements (IGAs) by October 1, 2026 to participate in the Ground Emergency Medical Transportation (GEMT) supplemental payment program for calendar year 2027. Even providers currently participating in 2026 must resubmit; new participants must include signed IGAs from authorized local government officials.
Action Required
By October 1, 2026: All government-owned ambulance transportation providers must complete and submit the cost report form (available on Department's Cost Reports webpage) to HFS.GEMT@illinois.gov. New 2027 participants must also obtain a signed Intergovernmental Agreement (IGA) from the authorized officer (Mayor, Village/Town Administrator, or Fire Protection District head) and submit both documents as Excel file and PDF with signatures. Providers should verify their authorized signatory is prepared to execute the IGA if this is their first year participating. Submission deadline is October 1, 2026. Failure to submit by deadline will result in ineligibility for GEMT supplemental payments in 2027, even if a provider currently receives them in 2026.
