Summary

Billing/Finance team should: (1) Determine preferred payment method (ACH Direct Debit or check); (2) If using ACH, request Form 3881 from HFS.GEMT@Illinois.gov; (3) If using check, make payable to Illinois Department of Healthcare and Family Services, include invoice number and note 'GEMT Payment', and mail to Bureau of Fiscal Operations – GEMT, P.O. Box 19491, Springfield, IL 62794-9491. This notice applies only to transportation providers, not clinical healthcare practices.
