Summary
Starting July 15, 2024, the new PO Boxes are mandatory: PO Box 240769 for claims, PO Box 240807 for adjustments and voids, and PO Box 240808 for provider enrollment and other mail. Effective September 1, 2024, if documents required to complete an application are not received within 15 days, new applications will be canceled and revalidation applications will be rejected. For incomplete applications, the enrollment team makes one outreach phone call and sends a follow-up email within two business days of receiving the application.

01Billing team should send claims to PO Box 240769, adjustments and voids to PO Box 240807, and provider enrollment and other mail to PO Box 240808, starting July 15, 2024.
02EFT update requests must include a readable copy of the front and back of the Authorized Representative's driver's license and a signature by an appropriate person.
03Electronic claims must go through a clearinghouse or billing agent that is actively enrolled with Alaska Medicaid or willing to enroll, because transactions from an entity that is not enrolled cannot be processed.
04For DSM submissions, use the DSM cover sheets, keep attachments to 5 maximum per email, and send any remaining documents in another email with Email 1 of 2 or similar in the subject line.
05Documents needed to complete an application must be received within 15 days, or new applications are canceled and revalidation applications are rejected.
