Action Required
Immediately: Update billing system to include new HCPCS codes Q9984, Q9985, Q9986, Q9989 for applicable provider types. Configure prior authorization requirement for C9498 in ASC and outpatient hospital settings. Remove discontinued codes C9487, J1725, P9072, 90649, 90650, 90748 from billing system. Update encounter forms and staff training materials. Claims using discontinued codes will be denied for dates of service after June 30, 2017.
