02Replace old IUD billing codes with new HCPCS codes W0001, W0002, W0004, and W0008 with updated fee schedules.
03Submit FD-999 Claims Adjustment Forms to request additional reimbursement for claims already paid at previous rates for dates of service on or after January 1, 1998.
