Billing team must ensure field 24H on HCFA 1500 claims includes family planning indicators: '2' for family planning services or '3' for beneficiaries under 21. Submit FD-999 Claims Adjustment Forms for any claims with dates of service on or after January 1, 1998 that were paid at previous rates to request additional reimbursement.
