Summary
New Jersey Medicaid/NJ FamilyCare is discontinuing four NJ-specific IUD billing codes (W0001, W0002, W0004, W0008) and replacing them with national HCPCS codes that separately bill for the insertion/removal procedure (CPT 58300/58301) and the device itself (J7300 for Paragard, J7302 for Mirena). Providers must update billing practices to use the new codes and complete the family planning indicator field on claim forms effective September 1, 2002.
Action Required
By September 1, 2002: Billing team must immediately discontinue use of codes W0001, W0002, W0004, and W0008 for all NJ Medicaid/NJ FamilyCare IUD claims. Update billing system to use CPT 58300 for IUD insertion (includes post-insertion visit) and CPT 58301 for IUD removal. Add J7300 (Paragard device) or J7302 (Mirena device) as applicable to each claim. On all CMS 1500 claim forms, complete block 24H (family planning indicator) with '2' for beneficiaries age 21+, or '3' for beneficiaries under 21. For device codes J7300/J7302, enter the AWP or acquisition cost (whichever is lower) in block 24F. Train all billing staff on the code transitions. Claims submitted with old W-codes will be denied; claims missing the family planning indicator or device codes will be rejected.
