Melodic Intonation Therapy (CPB 0284, reviewed 2026-04-17)
Aetna considers melodic intonation therapy medically necessary for Broca's (expressive, non-fluent) aphasia when all five selection criteria are met: good auditory comprehension; a well-motivated, emotionally stable member with a good auditory span; poor repetition, even for single words; no evidence of bilateral brain involvement; and non-fluent verbal production with diminished articulator agility and effortful initiation of speech. The policy lists melodic intonation therapy as experimental, investigational or unproven for dysarthria, other types of aphasia, verbal apraxia, neurogenic communication impairments secondary to Parkinson's disease and dementia, and raising trans women's singing and speaking fundamental frequencies, and it lists adjunctive transcranial magnetic stimulation or transcranial brain stimulation with melodic intonation therapy for aphasia as experimental too.
