(2) Update pre-authorization checklist to verify: member has good auditory comprehension, documented motivation/emotional stability/adequate auditory span, poor repetition even for single words, no evidence of bilateral brain involvement, and non-fluent verbal production with diminished articulator agility. (3) Reject and return claims that list ICD-10 codes F01.50-F03.C4 (dementia), F64.0-F64.9 (gender identity), G20.A1-G20.C (Parkinson's), G21.0-G21.9 (secondary parkinsonism), R47.1 (dysarthria), R48.1-R48.8 (apraxia), or R49.0-R49.9 (voice disorders) as these are explicitly non-covered. (4) Provider education: Distribute memo clarifying that MIT is covered ONLY for Broca's aphasia when all criteria are met; no coverage for dysarthria, Parkinson's disease-related communication disorders, dementia, other aphasia types, verbal apraxia, or combined TMS/MIT therapy. (5) Clinical staff must verify speech pathologist credentials and MIT training before authorizing. Claims lacking proper authorization or documentation will be denied with reference to CPB 0284.
