Before 2026-07-01: Billing team must implement the following controls: (1) Require prior authorization for all SGD claims (CPT 92607, 92608, 92609 and HCPCS E2500-E2513) by obtaining formal written SLP evaluation documentation before device ordering; (2) Create a checklist in the billing system to verify all required evaluation elements are present: functional communication goals, treatment plan with training schedule, assessment of natural communication alternatives, demonstration of cognitive/physical ability, impairment evaluation, device/accessory rationale, and physician notification; (3) Flag and deny claims for non-dedicated devices (desktops, laptops, tablets, smartphones) and multi-lingual modules as non-covered; (4) Deny claims for accessories without clear medical necessity documentation in the SLP evaluation; (5) Verify SLP performing evaluation has no financial relationship with device supplier and document this in claim file; (6) Enforce one SGD per member rule—deny duplicate or concurrent devices; (7) Deny separate billing for software installation, technical support, cables, adapters, and home modifications; (8) Update encounter forms and billing workflows to require SLP evaluation documentation before allowing SGD code submission. Consequences: Claims without proper SLP evaluation documentation will be denied; claims for non-dedicated devices will be denied; multiple SGDs per member will be denied.
