Excellus BlueCross BlueShield policy 2.01.52 says bioimpedance spectroscopy (BIS) may be considered medically necessary for diagnosis or surveillance of subclinical breast cancer-related lymphedema when all required criteria are met. The criteria require a history of breast cancer treatment with at least one listed factor, such as axillary lymph node dissection, sentinel lymph node biopsy with more than six nodes removed, regional node irradiation, taxane-based chemotherapy, sentinel lymph node biopsy with BMI greater than 30 kg/m2, or mastectomy in the setting of invasive breast cancer. BIS is considered investigational for all other indications. The January 22, 2026 off-cycle update added this statement and changed the policy title.

Aetna considers infrared coagulation medically necessary for anal dysplasia and for members with grade I, II, or III internal hemorrhoids that are painful or persistently bleeding. Low-level infrared light therapy is considered experimental, investigational, or unproven for a long list of indications, including osteoarthritis, migraines, stroke, and diabetes mellitus, and the infrared glove is unproven for Raynaud's syndrome and all other indications. CPT 46930 is covered when selection criteria are met, while CPT 97026 and HCPCS codes A4639 and E0221 are not covered for the listed indications.
Aetna clinical policy bulletin 0649 (reviewed September 14, 2026) considers extracorporeal shock-wave therapy medically necessary only for calcific tendinopathy of the shoulder that has lasted at least 6 months, has a calcium deposit of 1 cm or more, and has not responded to conservative care. It lists many other musculoskeletal, pain, wound and scar indications as experimental or not covered. The source gives a review date but no effective date, so confirm current applicability before changing workflows.
Aetna considers infrared coagulation medically necessary for anal dysplasia and for grade I, grade II, or grade III internal hemorrhoids that are painful or persistently bleeding. Infrared light treatment is considered medically necessary as a heat modality in physical therapy. Low-level infrared light therapy, including the Anodyne Therapy System, is considered experimental, investigational, or unproven for many indications such as diabetic and non-diabetic peripheral neuropathy, chronic non-healing wounds, lymphedema, migraines, and osteoarthritis, and the infrared glove is considered experimental for Raynaud's syndrome and all other indications.
Aetna considers home pneumatic compression devices medically necessary for members with chronic and severe lymphedema only after a documented four-week trial of conservative therapy (compliant compression, regular exercise and limb elevation) produces no significant improvement, and devices with calibrated gradient pressure only for lymphedema extending onto the chest, trunk or abdomen after a failed trial of a device without calibrated gradient pressure. Complex decongestive physiotherapy is considered medically necessary only for members with ulceration due to lymphedema, intractable lymphedema unrelieved by elevation, or prior admissions for complications of intractable lymphedema, who have a past record of compliance, and static compression sleeves are considered medically necessary for intractable lymphedema of the arms. Supplier prepared statements and physician attestations by themselves do not provide sufficient documentation of medical necessity, and Aetna considers acupuncture, kinesio taping, reverse lymphatic mapping and advanced pneumatic compression for head and neck edema experimental, investigational or unproven.
As last reviewed on July 9, 2026, Aetna considers fluidized therapy (fluidotherapy) medically necessary for the treatment of acute or subacute traumatic or non-traumatic musculoskeletal disorders of the extremities. Aetna considers it experimental, investigational or unproven for all other indications, including breast cancer-related lymphedema, carpal tunnel syndrome, limb edema after stroke, re-warming hypothermic subjects and rheumatoid arthritis. Fluidotherapy is a physical therapy modality subject to physical therapy guidelines and plan benefit limits, and Aetna covers only the professional charges of a physical therapist or other recognized, licensed provider for modalities that require direct, one-on-one patient contact.
Non-programmable single-compartment or multi-chamber pneumatic compression pumps applied to the limbs may be considered medically necessary for lymphedema that has failed to respond to conservative measures such as limb elevation and compression garments. Programmable pneumatic pumps applied to the limbs may be considered medically necessary only when the individual is otherwise eligible for a non-programmable pump and either has unique characteristics (such as significant scarring or recent surgery) that prevent satisfactory compression with a non-programmable pump, or has had an inadequate response to an initial course of treatment with a non-programmable pump applied to the limbs. Pneumatic compression pumps used to treat venous ulcers, and single-compartment or multichamber compression pumps applied to the head or neck, are considered investigational.
Liposuction for lipedema or lymphedema may be considered medically necessary to treat functional impairment when all required criteria are met. For lipedema, the criteria include absence of pitting edema from lipedema, bilateral and symmetrical manifestation with minimal involvement of the feet, disproportionate adipocyte hypertrophy of the extremities, a lack of effect of weight loss measures over six consecutive months, a negative Stemmer sign, and significant physical functional impairment. For lymphedema, the criteria include documented stage II or greater lymphedema on the International Society of Lymphology scale, other possible causes of edema ruled out, and no response to three to six consecutive months of conservative management, a period that was changed from six consecutive months on 10/08/2024.
Bioimpedance spectroscopy may be considered medically necessary to confirm a diagnosis of lymphedema in an individual who is asymptomatic, has a history of surgery, radiotherapy or trauma impacting the lymphatic system, and whose testing would guide early intervention such as physical therapy or complete decongestive therapy. It may also be considered medically necessary for surveillance in an asymptomatic individual with that history when testing would guide early intervention, or in an individual who remains symptomatic after a course of conservative therapy when testing would guide escalation such as liposuction or surgery. Bioimpedance spectroscopy is considered investigational outside these clinical scenarios, and the policy history records that the 10/01/2025 review changed the policy statement from investigational to medically necessary within them.
