Summary
Independence Blue Cross Medicare Advantage has changed bioimpedance spectroscopy (CPT 93702) coverage from Experimental/Investigational to Medically Necessary for lymphedema diagnosis confirmation and surveillance, effective 11/17/2025. Billing teams must now process claims for this service when specific medical necessity criteria are met, and must update documentation requirements to support coverage determinations.
Action Required
By November 17, 2025: Billing team must update billing system configuration to process CPT 93702 claims as covered (rather than automatically denying as experimental). Implement workflow to verify medical necessity meets one of two criteria: (1) asymptomatic patient with history of surgery/radiotherapy/trauma to lymphatic system where testing guides early intervention, OR (2) symptomatic patient post-conservative therapy where testing guides escalation. Add ICD-10 codes I89.0 and I97.2 to system as allowable primary/supporting diagnoses for CPT 93702. Update claim submission templates and provider education materials to reflect new coverage. Ensure medical records documentation requirements are met before claim submission—claims may be audited and denied if supporting documentation (office records, test reports, clinical notes) is unavailable upon Company request. Do not bill CPT 93702 for asymptomatic patients without relevant history or for symptomatic patients who have not completed conservative therapy, as these remain non-covered.
