Summary
Independence Blue Cross Medicare Advantage has changed coverage for bioimpedance spectroscopy (CPT 93702) from Experimental/Investigational to Medically Necessary for lymphedema diagnosis confirmation and surveillance in asymptomatic patients with lymphatic system injury or symptomatic patients after conservative therapy failure. Two ICD-10 diagnosis codes (I89.0, I97.2) have been added to support this coverage change.
Action Required
By November 17, 2025: (1) Billing team must update claim submission rules in billing software to change CPT 93702 from non-covered/investigational status to covered when ICD-10 codes I89.0 or I97.2 are present on the claim for Independence Blue Cross Medicare Advantage plans. (2) Update encounter forms and clinical documentation templates to include required medical necessity fields: confirmation of surgery/radiotherapy/trauma history affecting lymphatic system, and notation of how test results would guide early intervention decisions (for diagnosis) or therapy escalation decisions (for surveillance). (3) Providers must document one of the specific covered scenarios in patient medical records before billing. (4) Claims submitted without proper documentation of medical necessity will be denied. (5) Front desk and intake staff should flag all lymphedema-related visits for provider documentation review to ensure criteria are met before claim submission.
