Aetna considers full-leg or half-leg home-use pneumatic compression devices medically necessary durable medical equipment for chronic venous insufficiency of the legs in members with venous stasis ulcers that have failed to heal after a 6-month trial of conservative therapy directed by the treating physician. Intermittent pneumatic compression devices for the lower extremities are considered medically necessary for members who are bedridden due to trauma, orthopedic surgery, neurosurgery or other circumstances preventing ambulation, and a single patient use device is considered not medically necessary. Aetna lists other uses as experimental, investigational or unproven, including treatment of critical limb ischemia, intermittent claudication, post-thrombotic syndrome, restless legs syndrome and intra-operative compression for reduction of venous thromboembolism in craniotomy.

Aetna's clinical policy bulletin for carbon dioxide laser (CPB 0427) lists the uses it considers medically necessary: condyloma, pelvic pain associated with endometriosis, primary penile tumors (clinical stage Tis, Ta, and T1 Grade 1 to 2), refractory plantar warts, removal of actinic keratoses for members who meet CPB 0567 criteria, removal of superficial basal cell carcinomas, and fractional carbon dioxide laser for hypertrophic burn, traumatic, and surgical scars when conventional therapies have failed. Aetna considers carbon dioxide laser experimental, investigational, or unproven for a list of other indications, including hidradenitis suppurativa, male androgenetic alopecia, onychomycosis, port wine stains, stress urinary incontinence, and vaginal atrophy. Aetna considers carbon dioxide laser treatment of other actinic lesions cosmetic.
Non-programmable single-compartment or multi-chamber pneumatic compression pumps applied to the limbs may be considered medically necessary for lymphedema that has failed to respond to conservative measures such as limb elevation and compression garments. Programmable pneumatic pumps applied to the limbs may be considered medically necessary only when the individual is otherwise eligible for a non-programmable pump and either has unique characteristics (such as significant scarring or recent surgery) that prevent satisfactory compression with a non-programmable pump, or has had an inadequate response to an initial course of treatment with a non-programmable pump applied to the limbs. Pneumatic compression pumps used to treat venous ulcers, and single-compartment or multichamber compression pumps applied to the head or neck, are considered investigational.
Liposuction for lipedema or lymphedema may be considered medically necessary to treat functional impairment when all required criteria are met. For lipedema, the criteria include absence of pitting edema from lipedema, bilateral and symmetrical manifestation with minimal involvement of the feet, disproportionate adipocyte hypertrophy of the extremities, a lack of effect of weight loss measures over six consecutive months, a negative Stemmer sign, and significant physical functional impairment. For lymphedema, the criteria include documented stage II or greater lymphedema on the International Society of Lymphology scale, other possible causes of edema ruled out, and no response to three to six consecutive months of conservative management, a period that was changed from six consecutive months on 10/08/2024.
