Foot Bath Medications (New)
Humana's new Medicaid pharmacy guidance (effective November 5, 2026) states that select medications such as budesonide, ciclopirox, clindamycin, fluocinonide, ketoconazole, linezolid, nystatin, and vancomycin, when dispensed for mixing into foot baths or soaks, are generally not medically necessary and may lack compendia or label support. Claims for these drugs submitted close together with other foot bath medication claims may reject with NCPDP error code 88 / internal code 527 ('Questionable compendia support'), and medications are approved only if label or compendia-supported use is shown on clinical review.
