By March 30, 2022 (180 days): For fee-for-service claims, submit with bulletin attachment. Update billing system to reflect that most new codes (0018M, 0255U-0265U) are noncovered by Indiana Medicaid. Set Q0240 to zero payment as monoclonal antibody is provided at no charge.
