Autotransfusers and Red Blood Cell Genotyping (CPB 0639, reviewed 2026-09-09)

Effective date

Summary

Aetna considers autotransfusion and cell saver devices medically necessary for emergency or intra-operative autotransfusion, hemodilution or cell washing autotransfusion, and post-operative autotransfusion (usually done within 2 hours with a chest tube collection device) after heart surgery and traumatic hemithorax. Aetna considers these devices experimental, investigational, or unproven for all other indications, and not medically necessary for procedures expected to require less than 2 units of blood. Red cell genotyping is medically necessary for sickle cell disease, thalassemia, hemoglobinopathies, recurring transfusions, post-transfusion hemolysis with no detectable antibodies, autoimmune hemolytic anemia, multiple transfusions, positive direct antiglobulin test, certain pregnancies, hemolytic disease of the fetus and newborn, and conflicting serological antibody results, while hypoxic red blood cells are considered experimental, investigational, or unproven for improving energy metabolism and post-transfusion recoveries.

What to do

Action · 6 steps
01Billing team: Bill CPT codes 86890 and 86891 only when the selection criteria in this policy are met, since they are covered only if those criteria are met.
02Use autotransfusion or cell saver devices only for the listed indications, because they are not considered medically necessary for procedures expected to require less than 2 units of blood.
03Do not use autotransfusion on blood exposed to bacteria, such as an infected wound or blood with fecal contamination, or on blood with malignant cells.
04Expect HCPCS P9027 to be not covered for the indications listed in this policy.
05Bill the red cell genotyping codes in this policy only when one of the listed situations applies, such as sickle cell disease, thalassemia, recurring transfusions, multiple transfusions, or conflicting serological antibody results.
06Do not bill hypoxic red blood cells as a covered service, because the policy considers them experimental, investigational, or unproven.
