Autotransfusers and Red Blood Cell Genotyping (CPB 0639, reviewed 2025-12-11)

Aetna's Policy 0639 (effective 2025-12-11) establishes medical necessity criteria for autotransfusion/cell saver devices and red blood cell genotyping. Autotransfusers are covered only for high-blood-loss procedures (≥2 units), emergency/intra-operative salvage, or post-operative collection. Red cell genotyping is covered for transfusion-dependent hemolytic conditions, post-transfusion hemolysis, autoimmune hemolytic anemia, and fetal hemolytic disease management. All other uses are considered experimental.
