Enhanced External Counterpulsation (EECP) (CPB 0262, reviewed 2026-05-14)

Effective date
Not stated in the notice
Identified
Aug 15, 2026

Summary

Aetna considers EECP medically necessary for chronic stable angina as a single course of 35 treatment hours (CPT 92971 or HCPCS G0166) for members with disabling Canadian Cardiovascular Society Class III or IV angina who are refractory to maximum medical therapy and not readily amenable to PTCA or cardiac bypass. A repeat course is medically necessary when the member still meets the single-course criteria, prior EECP produced a sustained improvement (a greater than 25% reduction in anginal frequency or an improvement of one or more anginal classes), and at least three months have elapsed since the prior treatment. Hydraulic versions of these devices are considered not medically necessary, and EECP is considered experimental, investigational or unproven for conditions such as heart failure, erectile dysfunction, stroke, long COVID, microvascular angina and restless leg syndrome.

What to do

Action · 5 steps
01Billing team: Claims for CPT 92971 or HCPCS G0166 need documentation that the member meets the single-course criteria, including disabling chronic stable angina at CCS Class III or IV, refractory to maximum medical therapy, and no present comorbid conditions or contraindications that would result in excessive risk.
02A single course is defined as 35 treatment hours, usually given as 35 one-hour sessions five days a week, though two sessions may be completed in one day if tolerated, and there is no proven benefit to extending a course beyond 35 sessions.
03A repeat course needs documentation that the prior course produced a sustained improvement and that three or more months have elapsed since that treatment.
04Contraindications listed in the policy include acute decompensated heart failure, recent myocardial infarction within the last 3 months, unstable angina pectoris, severe hypertension greater than 180/110 mm Hg, coagulopathy with INR greater than 2.0, and any surgical intervention within 6 weeks before EECP.
05ICD-10 codes listed as contraindicated for this policy include I21.01 through I21.B, I26.01 through I26.99 and I48.0 through I48.92.
