Summary
Aetna has updated its Ambulatory Electroencephalography (EEG) medical policy (CPB 0425) effective July 7, 2026. The policy clarifies medical necessity criteria for ambulatory EEG with or without home video monitoring, specifies that monitoring beyond 7 days requires medical necessity review, and explicitly designates implantable sub-scalp EEG monitoring systems as experimental/investigational and not covered. Billing teams must ensure claims for ambulatory EEG meet the three specified clinical criteria and that prior authorization is obtained for monitoring exceeding 7 days.
Action Required
By July 7, 2026: (1) Billing team must update authorization rules in claims system to flag ambulatory EEG claims (CPT 95700-95726) requiring verification of one of three medical necessity criteria: seizure type classification, seizure disorder diagnosis, or pre-surgical epileptogenic region localization. (2) Implement system requirement to verify recent neurologic examination and standard EEG studies within 12 months prior to ambulatory EEG (exception: ICU continuous EEG waives this requirement). (3) Set automated denial flag for ambulatory EEG monitoring exceeding 7 days without documented medical necessity review and prior authorization. (4) Deny all claims for implantable sub-scalp EEG codes (0956T, 0957T, 0958T, 0959T, 0960T, 1004T, 1005T, 1006T, 1007T, 1008T) as these are non-covered/experimental. (5) Update encounter templates and pre-authorization forms to require providers document which of the three covered indications applies and date of most recent neurologic exam. (6) Communicate denial/rejection logic to providers and credentialed staff. Claims submitted without qualifying criteria will be denied.
