Summary

Aetna considers amnioinfusion medically necessary for reducing variable decelerations of the fetal heart rate caused by cord compression during labor, and for intrauterine resuscitation in the setting of a Category II intrapartum fetal heart rate tracing to improve FHR abnormalities before cesarean delivery. Aetna considers amnioinfusion experimental, investigational or unproven for indications including chorioamnionitis, facilitation of external cephalic version, preterm premature rupture of membranes, prevention of meconium aspiration syndrome, prevention of postpartum hemorrhage, prophylactic treatment of oligohydramnios, and treatment of anhydramnios from bilateral renal agenesis. CPT 59070 (transabdominal amnioinfusion, including ultrasound guidance) is covered if selection criteria are met.

What to do

Action · 3 steps
01Billing team: bill CPT 59070 (transabdominal amnioinfusion, including ultrasound guidance) only when the selection criteria are met, meaning for reduction of variable fetal heart rate decelerations because of cord compression during labor, or for intrauterine resuscitation with a Category II intrapartum fetal heart rate tracing before cesarean delivery.
02Pair the service with a covered ICD-10 code for that indication, such as O76 (abnormality in fetal heart rate and rhythm complicating labor and delivery) or the labor and delivery umbilical cord complication codes O69.0xx0 through O69.2xx9.
03Aetna considers amnioinfusion for the experimental indications in this bulletin experimental, investigational or unproven, and the ICD-10 codes listed as not covered for those indications include O41.00x0 through O41.00x9 (oligohydramnios) and O42.00 through O42.92 (premature rupture of membranes).
