Summary
Effective September 1, 2026, Medi-Cal hospice providers must transition from submitting the Hospice Program Attestation Form via web to using the Medi-Cal Provider Portal. Attestation forms must be submitted within 5 calendar days of member hospice election, and the signed DHCS 8052 form must be retained in the medical record. A mandatory training webinar is scheduled for August 26, 2026.
Action Required
By August 26, 2026: Hospice billing and clinical staff must register for and attend the DHCS live training webinar (HH102LW) on the Medi-Cal Provider Portal Hospice Attestation process through the Medi-Cal Learning Portal (MLP). By September 1, 2026: Billing team must implement new workflow to submit all Medi-Cal Hospice Program Attestation Forms through the Medi-Cal Provider Portal within 5 calendar days of member hospice election. Clinical staff must ensure the signed DHCS 8052 form is obtained, retained in the member's medical record, and made available for audit requests. Update internal procedures and staff training documentation to reflect the portal transition. Non-compliance will result in missing attestations and potential claim denials or audit findings.
