01Hospice billing and clinical staff must register for and attend the DHCS live training webinar (HH102LW) on the Medi-Cal Provider Portal Hospice Attestation process through the Medi-Cal Learning Portal (MLP).
02By September 1, 2026: Billing team must implement new workflow to submit all Medi-Cal Hospice Program Attestation Forms through the Medi-Cal Provider Portal within 5 calendar days of member hospice election.
03Clinical staff must ensure the signed DHCS 8052 form is obtained, retained in the member's medical record, and made available for audit requests.
