Summary

Effective September 1, 2026, Medi-Cal hospice providers must submit the Hospice Program Attestation form through the Medi-Cal Provider Portal instead of the current web-based form. The old form will no longer be accessible. Providers without Portal access will be unable to submit attestations and may experience delays.

By August 31, 2026: (1) Billing team and hospice providers must verify Medi-Cal Provider Portal account access and registration status. (2) Providers currently billing Medi-Cal fee-for-service should confirm existing Portal credentials work. (3) New Medi-Cal hospice providers or those without Portal access must register immediately at the Medi-Cal Providers website to avoid submission delays. (4) All staff involved in attestation submission should attend the mandatory training webinar on August 26, 2026 (11:00 a.m. - 12:30 p.m.) by registering through the Medi-Cal Learning Portal. (5) Update internal workflows and checklists to reflect the new Portal-based submission process. Failure to register and transition by September 1, 2026 will result in inability to submit attestations and potential compliance issues with Medi-Cal.
