01All current Illinois Medicaid providers must revalidate their enrollment in the new IMPACT system using their mailed Application ID.
02New providers can enroll starting August 3, 2015.
03Billing team must ensure all required documents are ready: NPI number, certified W-9 form, taxonomy number, current licenses/certifications, and valid email address.
