By April 1, 2026: Billing team must update system to flag all proprietary laboratory codes 0614U-0630U as non-covered for BCBSM plans. By July 1, 2026: Update system to flag all Category III codes 1026T-1053T as non-covered. Do not submit claims for these codes to BCBSM as they will be denied. Verify coverage with other payers before using these codes.
