Summary
Blue Cross Blue Shield of Michigan has added 29 new proprietary laboratory analysis codes (0631U–0659U) and 2 new vaccine codes (90616, 90639) effective July 1, 2026. Coverage varies by code: most are not covered, two laboratory codes (0632U, 0657U, 0658U, 0659U) are covered, one code (0648U) requires manual review, and both vaccine codes are not covered. Billing teams must immediately update their systems to reflect these coverage decisions to avoid claim denials.
Action Required
By July 1, 2026: Billing team must update billing software and claim processing system to recognize the 31 new CPT codes (0631U–0659U, 90616, 90639) and apply the correct coverage determinations per BCBS of Michigan. Specifically: (1) Configure system to deny claims for codes 0631U, 0633U–0647U, 0649U–0656U, 90616, and 90639; (2) Configure system to allow/cover claims for codes 0632U, 0657U, 0658U, and 0659U; (3) Configure code 0648U to flag for manual review before processing. Update claim scrubbing rules and denial templates. Notify providers and laboratory staff of non-covered codes to prevent unnecessary testing. Failure to implement these changes will result in claim denials, delayed reimbursement, and patient billing disputes.
