01Providers operating as PXRS or IDTF must re-enroll in Michigan CHAMPS system as appropriate specialty type within 6 months of effective date.
02Submit requests via fax to 517-241-8233 or email to providerenrollment@michigan.gov.
03Billing team must ensure Medicare enrollment is completed before applying to Michigan Medicaid.
04Update billing procedures to use proper HCPCS codes R0070, R0075, Q0092 with appropriate modifiers for portable x-ray services.
