Summary
This policy establishes coverage criteria for MGMT promoter methylation testing in malignant glioma patients. The test is medically necessary only for patients with high-grade malignant gliomas who are candidates for temozolomide therapy or radiotherapy, and when results will direct treatment decisions.
Action Required
Immediately: Billing team must verify that MGMT promoter methylation testing claims include documentation showing: 1) Patient has high-grade malignant glioma (glioblastoma multiforme or anaplastic astrocytoma), 2) Patient is candidate for temozolomide therapy or radiotherapy, and 3) Test results will be used to direct therapy choices. Claims not meeting all three criteria will be denied as investigational.
