Summary

This policy establishes coverage criteria for Magnetoencephalography/Magnetic Source Imaging (MEG/MSI) procedures. MEG/MSI is considered medically necessary for determining language function laterality as a substitute for the Wada test in pre-surgical planning, and for evaluating drug-resistant epilepsy when standard techniques don't provide adequate localization. All other uses are considered investigational and not covered.

Ensure prior authorization requests include documentation of: 1) Pre-surgical evaluation for epilepsy, brain tumors, or other brain resections requiring language laterality determination, OR 2) Drug-resistant epilepsy with inadequate localization from standard MRI and EEG techniques. Claims for other MEG/MSI indications will be denied as investigational. Update encounter forms and provider documentation requirements accordingly.
