Summary
This policy establishes coverage criteria for radiofrequency ablation (RFA) of tumors outside the liver. RFA is now considered medically necessary for specific conditions including osteolytic bone metastases, osteoid osteomas, small renal cell carcinomas in poor surgical candidates, and small lung tumors when surgery/radiation aren't viable options.
Action Required
Immediately: Review all pending and future RFA procedures to ensure they meet the specific medical necessity criteria outlined in this policy. Document patient eligibility including tumor size limits (≤4cm for renal, ≤3cm for lung), surgical candidacy status, and anatomical location requirements. Update prior authorization requests to include required clinical justifications. Claims for RFA procedures not meeting these criteria will likely be denied.
