Summary
Connecticut Medicaid (CMAP) has implemented 2025 HCPCS coding updates effective October 1, 2025, with new ICD-10 codes (156 surgical, 487 diagnosis) being added and some discontinued. Inpatient claims with new codes will suspend until the updated 3M DRG grouper is loaded around October 28, 2025.
Action Required
Immediately: Monitor inpatient claims with header Through Date of Service October 1, 2025 and forward for suspensions with EOB codes 0693 or 0920. Claims will auto-reprocess once the 3M Grouper updates around October 28, 2025. Update Medical Nutrition Therapy billing to reference updated Table 26 diagnosis codes (retroactive to July 1, 2025). For CHNCT inpatient elective authorizations, contact CHNCT at 1-800-440-5071 if admission dates change to avoid claim denials.
