New Jersey Medicaid/NJFamilyCare is separating IUD billing into two components: procedure codes for insertion/removal (CPT 58300, 58301) and device codes (HCPCS J7300, J7302). Eight legacy state-specific codes (W0001-W0008 series) are discontinued. Providers must update billing systems and claim forms to use the new code set effective September 1, 2002.

By September 1, 2002: (1) Billing team must immediately discontinue use of legacy codes W0001 WF, W0001 WF WM, W0002 WF, W0002 WF WM, W0004 WF, W0004 WF WM, W0008 WF, and W0008 WF WM in the billing system. (2) Update billing software to use CPT 58300 (IUD insertion) and 58301 (IUD removal) with modifier WF and appropriate follow-up days. (3) Configure system to bill IUD devices separately using J7300 (Paragard) or J7302 (Mirena) codes. (4) Update CMS 1500 claim form templates to populate block 24H with family planning indicator '2' (age 21+) or '3' (under 21). (5) Train providers to enter Average Wholesale Price (AWP) or acquisition cost in block 24F for device codes. (6) Verify billing system recognizes indicator 'E' exclusion from multiple surgery pricing rules. Failure to update claims will result in denials for IUD procedures submitted under old codes.
