Summary
Effective May 1, 2026, CareSource is transitioning routine vision benefit administration from Versant/Superior Vision to EyeMed for all Indiana Medicaid members. Billing teams must route claims to EyeMed (P.O. Box 8504, Mason, OH 45040; EDI Payer ID 31165) for dates of service on or after May 1, 2026, while continuing to submit pre-May 1 claims to Versant/Superior Vision. Medical vision benefits remain with CareSource.
Action Required
By April 30, 2026: Billing team must update claim routing procedures and billing system configuration to direct all routine vision claims with dates of service May 1, 2026 or later to EyeMed Vision Care (P.O. Box 8504, Mason, OH 45040; EDI Payer ID 31165). Verify EyeMed online claims system access and member benefit details. Update internal documentation and provider notices to reflect the vendor change. For dates of service on or before April 30, 2026, continue submitting claims to Versant/Superior Vision per existing procedures. Failure to route claims to the correct vendor will result in claim denials or processing delays.
