Summary
Effective January 1, 2026, EyeMed replaces Versant/Superior as the routine vision vendor for Ohio Medicaid and CareSource MyCare Ohio (HMO D-SNP). Vision providers must contract with EyeMed to deliver routine vision services to these members, while medical vision services still require CareSource contracts. Claims with service dates through December 31, 2025, will continue processing through the old vendors until December 31, 2026.
Action Required
By December 15, 2025: Billing team must identify all routine vision claims for Ohio Medicaid and CareSource MyCare Ohio (HMO D-SNP) members and verify EyeMed network participation status. Contact EyeMed Provider Services at 1-877-857-2402 to confirm contracting and obtain updated provider network information, fee schedules, and claims submission procedures. Update billing system routing rules to direct routine vision claims for these plans to EyeMed effective January 1, 2026. Maintain separate contracting status for medical vision services, which continue under CareSource. Train billing staff on new EyeMed call center number and claims processes. Failure to establish EyeMed contracts will result in claims denial for routine vision services provided to eligible members on or after January 1, 2026.
