Immediately: Update record retention policies to keep all records dated December 1, 2015 or later for 7 years instead of 6 years. By February 1, 2022: Ensure all home and community-based services requiring EVV have complete verification records - claims without EVV will be denied starting this date. For Emergency Medicaid dialysis claims after January 1, 2022: Use Condition Code 74 for home services and mark emergency fields (24C for CMS-1500, Admission Type 1 or 5 for UB-04).
