Dental Crowns and Veneers 13.01.02
The dental crowns and veneers policy (13.01.02) has a current effective date of July 16, 2026, and that annual review left the policy intent unchanged. Traditional or three-quarter crowns are medically appropriate for a large filling covering at least half the tooth width, after a root canal to prevent fracture, for cracked tooth syndrome with pain upon chewing, for a tooth missing a facial/buccal or lingual/palatal wall, or for severe decay that destroyed most of the original tooth. Veneers on anterior teeth 6-11 or 22-27 are medically appropriate only for a large filling covering at least half the tooth width or following a root canal to prevent fracture, and crowns placed to cover a misshaped or severely discolored tooth, or veneers placed for severe discoloration, wear, misalignment, gaps, cracked tooth syndrome, a broken cusp or severe decay, are not medically necessary.
