Summary
L.A. Care Health Plan is offering a $7,000 incentive program for skilled nursing facilities to adopt Health Information Exchange (HIE) systems through LANES or CMT/PCC platforms. By June 17, 2024 at 5pm: Eligible skilled nursing facilities with L.A. Care contracts must submit applications via fillable PDF format to strategicinvestments@lacare.org if invited to participate. Complete three milestones within 12 months: execute CalHHS Data Sharing Agreement, finalize Business Associate Agreement with LANES or CMT/PCC, and exchange ADT and CCD documents. Submit progress reports by August 1, 2024 and within 30 days of incentive term end.
