01Billing team must update system to require preauthorization for codes C9069, C9070, C9072, C9073, J1823, J7352, J9144, J9223, J9316, J9317 (all plans) and Q5122 (commercial only).
02Stop billing C9071 and C9771 as they are not covered.
03Remove codes G2214, G2250, G2251, G2252 from institutional billing as they are not separately reimbursed.
04Update encounter forms and notify providers about non-covered services before rendering them.
